Chronic Disease Management Classes
Diabetes 101 – Led by a registered nurse, this free, one-hour session provides an introduction to type 2 diabetes, including prevention, risks and management strategies. Go beyond the exam room to gain the insight and understanding you need to control insulin levels and reduce the risk of life-threatening complications.
Living Well With Diabetes – Our free Living Well with Diabetes workshop, developed by researchers at Stanford University, is open to anyone with a clinical diagnosis of diabetes. Over six sessions, you will develop weekly action plans as you learn medication management, symptom control and tips to live a full and active life.
Carb Counting– This free class empowers you to take control of your blood sugar by understanding how carbohydrates affect your body, how to identify and avoid carb-rich foods, what foods can lower blood sugar and more. The class is led by a nutritionist and board certified health and wellness coach.
Nutrition for Diabetes – Led by Get Healthy, Live Well’s registered and licensed dietitian, this free one-hour session, gives participants a variety of examples and tips for health eating which benefits the overall nutrition in managing diabetes. From eating at home, on the go and best foods to order in restaurants, learning nutrition values will help guide you in making the right choices and live a healthier life with diabetes.
Living Well Workshop - Our free Living Well Workshop through Stanford University is for those living with ongoing symptoms of chronic disease, such as fatigue, pain, tense muscles or shortness of breath. The six-session Living Well Workshop will help you manage any chronic disease, including arthritis, cancer, depression, diabetes, heart disease, hypertension and more. Family members and informal caregivers are also encouraged to attend classes.
Prevention and Wellness Classes
Diabetes Prevention Program– The Diabetes Prevention Program, designed by the Centers of Disease Control and Prevention (CDC), is a series of free classes that can help you make lasting changes to fight the key risk factors of developing diabetes. The program combines coaching from a certified lifestyle educator, proven learning materials and small-group support to set you on the path to better health.
Tai Chi for Health – We are offering training in this ancient Chinese martial art to adults. Based on Sun and Yang style of tai chi, classes combine deep breathing and relaxation with slow, gentle movements. Upon completing the six-week beginners Tai Chi session, participants are invited to advance their training and move into Intermediate Tai Chi and then onto Advanced Tai Chi. Both the Intermediate and Advanced Tai Chi are six weeks long and cost $45 for each series.
Freshstart – Want to quit tobacco, vaping or nicotine? This free, one-hour class meets for four weeks. It is designed to empower participants with tools and resources to quit the habit for good with the support you need.
Moving On Mondays – In partnership with the West Georgia Track Club in Carrollton, Tanner's Get Healthy, Live Well offers an eight-week program designed to turn walkers into joggers. In a group setting, participants learn skills and strategies to help them gradually pick up the pace, so they can graduate from walker to jogger to 5K aficionado. Dietitian Services
With registered dietitians on staff, Get Healthy, Live Well is able to offer one-on-one nutrition education to patients needing help controlling their weight, blood pressure, cholesterol, diabetes and other chronic conditions.
