Medical Records Request

 

Tanner Health System safely and responsibly collects, maintains and provides quality health information for our patients. We also are committed to providing accurate patient information to our customers in an efficient and timely manner, while protecting the confidentiality and security of that information.

Tanner Health System is pleased to offer the following options for requesting and receiving your medical records:

  1. Using the Tanner MyChart portal
  2. Completing a self-serve online medical records request
  3. Printing an authorization form and using email/fax/mail

Access your medical records with Tanner MyChart

Tanner MyChart allows you to review portions of your medical/surgical history, including lab results, test results, current immunizations and provider notes.

You may also request the remaining portions of your medical records through MyChart. It is free to use, secure and available 24 hours a day. Please allow up to 3 to 5 business days for your records to be processed and released.

If you don't have a Tanner MyChart account, you can create one now.

 

Request copies of your medical records via online authorization

Medical records can be released to a patient (or anyone that the patient authorizes) by completing an online medical records authorization.

It may take 7 to 10 business days to process your request. Depending on the scope of the request, a fee may be charged.

To request your medical record electronically, you must have a valid e-mail address, a cell phone and a driver’s license (or government issued photo ID) available to verify your identity.

 

Request copies of your medical records with a printed form

Please send a completed and signed medical records authorization printed form to the Tanner Health Information Management Department:

Email: himroi@tanner.org (attach a scan of the completed form)
Fax:  770-812-9206 (attach a scan of the completed form)
Mail: Attn: HIM Dept/Medical Records, 705 Dixie Street, Carrollton, GA 30117 (enclose your completed form)

The medical records authorization MUST contain the following information or your request cannot be processed:

  • Patient's full name, date of birth, address, phone number (include any previous names)
  • Person, attorney or physician to whom the information is to be sent (name and complete address)
  • The specific information being requested (e.g., location of treatment, provider name, type of report/information and dates of service, etc.)
  • Signature of the patient or the patient's legal representative. (Authorizations signed by a representative must contain a copy of the guardianship papers or power of attorney.)
  • Date of the signature

 Obtain copies of birth and death certificates

To obtain a copy of a birth or death certificate, you must contact the county clerk’s office in the county the birth or death occurred or from the Georgia or Alabama Department of Public Health.
