Chronic Disease Management

Primary care plays an important role in helping patients manage ongoing, long-lasting health conditions known as chronic diseases. Unlike temporary illnesses, these conditions require consistent management through treatment, monitoring and healthy lifestyle choices.

Our providers care for patients with chronic illnesses such as diabetes, high blood pressure (hypertension), heart disease, chronic obstructive pulmonary disease (COPD), chronic kidney disease (CKD), obesity, arthritis and memory-related conditions like Alzheimer’s disease and dementia.

Our approach combines primary care, care management and technology to proactively manage chronic conditions, reduce hospital visits and improve quality of life.

Care focuses on prevention, symptom control and coordination across providers — helping patients stay as active, healthy and independent as possible. For qualifying patients who need additional support between visits, Tanner Primary Care also offers Chronic Care Management (CCM) — an added layer of care beyond routine visits.

Chronic Care Management program

While primary care visits are essential, managing a chronic disease doesn’t stop when you leave the doctor’s office. That’s why Tanner Health offers Chronic Care Management (CCM) — a structured, ongoing program for patients living with multiple long-term conditions that extends care beyond routine visits.

CCM helps connect the dots across your care. Through this program, our care team works with you to monitor your health, manage medications, coordinate care and answer questions — all outside of regular appointments.

Your care team includes nurses, care coordinators and other clinical support staff working alongside your primary care provider.

You may qualify for CCM if you have two or more chronic conditions that are expected to last at least 12 months and require ongoing medical treatment, such as diabetes, high blood pressure, COPD, chronic kidney disease or heart disease.

CCM may include:

  • A personalized care plan based on your health needs and goals
  • Regular monthly check-ins with a dedicated member of your care team
  • Medication review and support
  • Care coordination with specialists, hospitals, pharmacies and community resources
  • Help navigating symptoms or changes in your condition
  • 24/7 access to clinical support when you need it

An initial face-to-face visit with your provider is required before CCM services begin, and patient consent is needed to enroll.

CCM is a Medicare-supported service. Services may include a small monthly copay depending on insurance coverage. Participation is optional, and you can choose to stop at any time.
