Preparing for your visit

For your convenience, please print and fill out our registration form to reduce wait times during your first visit.

During your initial consultation, we will discuss your treatment options and review your:

  • Current medical history

  • Current medications and/or supplements

  • Diet and exercise regimen

  • Previous nicotine exposure

Remember to bring a current photo ID and insurance card with you. We accept most insurance plans when applicable and we also offer CareCredit, the credit card for health, beauty and wellness.
